Parents Please Be Aware!  Your 5th grade student will need additional immunizations to enter middle school.

The 2019-2020 requirements for students entering 6th grade are as follows:

*3 Hep B         *2 Varicella

*5 DTaP          *1 Tdap (Tetanus & Pertussis)

*4 Polio           *1 MCV4 (Meningococcal)

*2 MMR           *2 Hep A

Most students will need the additional Tdap & MCV4, some may need the Hep A.  If you are unsure of what your child needs, please contact the school clinic at 260-431-2618 or contact your provider directly.
