The information provided on these pages may help you make a decision, and help you better describe your child's speech and language when you contact us.
If you have not yet contacted the Early Childhood Coordinator regarding an evaluation for your child(ren) you can call (260) 431-2041.
What to bring for the initial speech and language evaluation:
Your child! And….
- your child’s birth certificate,
- their current immunization records,
- proof of residence (this can be a utility bill, a rental agreement, or other document mailed to you at your address)
Coming from First Steps?
Your service coordinator will set up a transition meeting with the Early Childhood Coordinator here at Southwest Allen County Schools. Following your transition meeting a time for an evaluation will be set up.
Plan to set aside one hour for the initial speech and language evaluation.
When you arrive at 415 Mission Hill Dr. (Whispering Meadows Elementary School), please park in the visitor parking lot (the north parking lot) in front of the playground. The lot directly in front of the school is a faculty lot only.
